Long-Term Care Quality Improvement Community of Practice (Ontario)

January 2025 to November 2025

This action-oriented Community of Practice (COP) was targeted towards interdisciplinary health care professionals who work in long-term care settings in Ontario and who were actively involved in palliative care-related quality improvement initiatives within their organizations.

This COP aimed to support these individuals in the planning and successful implementation of palliative care-related projects falling under their provincially mandated Quality Improvement Plans (QIPs). COP members had the opportunity to share lessons learned from previous QI projects, gain feedback from their peers on future plans, receive advice from content/ context experts and build shared resources to support their QI work.

Through an equity-focused lens and practice-level case-based discussions, participants worked together to find ways to bolster baseline practices that support good care for everyone.
